ULM Athletics Event Participation Waiver and Media Release
By purchasing tickets, registering for, attending, or participating in any University of Louisiana Monroe Athletics event, promotion, program, experience, or activity (collectively, the “Activities”), whether for myself, my child, or another individual for whom I am legally authorized to act, I acknowledge and agree to the following:
Assumption of Risk and Release of Liability
I understand that attendance at and participation in the Activities is voluntary and may involve inherent risks, including but not limited to slips, falls, collisions, contact with participants or spectators, interactions with student-athletes, coaches, staff, mascots, or equipment, field or facility conditions, weather conditions, promotional activities, and other risks that could result in injury, illness, property damage, or death.
On behalf of myself and any minor child for whom I am authorized to act, I voluntarily assume all risks associated with attending or participating in the Activities and, to the fullest extent permitted by law, release, waive, and hold harmless the University of Louisiana Monroe, the University of Louisiana System, the State of Louisiana, the ULM Athletic Foundation, and their respective trustees, officers, employees, coaches, student workers, volunteers, agents, sponsors, contractors, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or relating to attendance at or participation in the Activities, except to the extent caused by the gross negligence or willful misconduct of a Released Party.
Indemnification
I further agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, demands, damages, losses, liabilities, costs, or expenses (including reasonable attorneys’ fees) arising out of or relating to my own or my child’s attendance at or participation in the Activities, except to the extent resulting from the gross negligence or willful misconduct of a Released Party.
I represent that I am physically able, and that any minor child for whom I am acting is physically able, to safely attend or participate in the Activities. If I am accepting these terms on behalf of a minor, I certify that I am the parent or legal guardian, or otherwise legally authorized to do so.
Photo and Media Release
I grant the University of Louisiana Monroe and ULM Athletics permission to photograph, film, record, livestream, or otherwise capture my and/or my child’s name, image, likeness, voice, and appearance while attending or participating in the Activities.
I authorize ULM Athletics to use, reproduce, publish, distribute, display, edit, modify, and otherwise use such materials, without compensation or further approval, for lawful promotional, educational, marketing, fundraising, recruiting, archival, and publicity purposes in any media now known or later developed, including but not limited to social media, websites, digital content, broadcasts, streaming platforms, printed materials, and other University publications.
I understand that such materials may be edited or combined with other content, and I waive any right to inspect or approve the finished product.
